Patient Forms
To make your experience at Alaska Colorectal Surgery as easy and seamless as possible, all of the forms you need have been added to
this site for you to print out, fill out on-line and bring into the office.
Please fill out the following
forms prior to you being seen. They will give us the needed information to better care for you, as well as save you time
on the day of your appointment.

AFTER HOUR CALLS:
If you have a medical situation that cannot wait until the next business day, call our
regular phone number and you will get our answering service. They will call the “on call” doctor. If you have not
received a return call within an hour, call again. Sometimes errors are made in the phone numbers or in the paging. We
want you to be helped quickly and efficiently.
Please call again and let the answering service know it has been over an hour and you have not received a call. If your situation is a true emergency, please go straight to the emergency room.
